CLAIMS ONLY FOR PERMANENT STAFF
REIMBURSEMENT OF OPD AND INDOOR CLAIMS
 

Reimbursement for OPD treatment, when referred to these hospitals be as under

1. Consultation fees in the above hospital will be reimbursed in full.
2. Routine investigation of blood serum, urine, stool, ECG etc – 80% of the charges.
3. Special investigations like Gastroscopy, Endoscopy, Ultrasonography etc. 80% of the charges



REIMBURSEMENT OF INDOOR TREATMENT
 

* In case of claims beyond Rs.15,000/-the institute shall reimburse first Rs.15,000/- in full and only 80% of the admissible amount over and above the first 15,000/- . The remaining 20% will have to be borne by the staff member.

* When staff members are referred to these Hospitals for indoor treatment, accommodation should be sought in the entitled class only. In case where higher class is availed, the reimbursement will be restricted to the entitled accommodation. In specific cases where the accommodation in the entitled class is not available and the admission could not be postponed till the accommodation in the entitled class is available, without causing danger to the life of the patient.

* In such cases accommodation in the next higher class will be considered. Similarly, in cases where the condition of the patient is so serious that accommodation in a special room / ICCU becomes inevitable, such charges will be reimbursed on the basis of the certificate issued to this effect by the concerned specialits/Hospital authorities.

* Patients residing outside IIT Campus, near these Institutions may obtain facilities for permanent reference under the following conditions :-

a) Any one hospital near their residence recognized by IIT for full reimbursement can be selected by the employee except for Dombivli area where both Manav Kalyan Kendra and Dhanwantari Hospital can be selected as Dhanwantari Hospital offers only OPD facility.(Application forms are available in the Hospital Office).
b) Reimbursement is allowed for regular OPD only at the said hospital for family members and in case of emergency, for the employee. Investigations prescribed by IIT Hospital that are to be done on empty stomach and other investigations if co-prescribed are reimbursable for the employee as well.
c) If investigations are advised repeatedly, IIT Hospital Medical Officer should be informed before hand. Expensive investigation such as C.T. Scan and MRI Scan, if advised have to be done in the recognised Centres after taking reference note from Medical Officer of IIT Hospital, unless in emergency.
d) Reference from the selected hospital to any other hospital will not be considered for reimbursement except in emergency.
e) Reimbursement will be at par with the IIT Rules.
f) No T.A. is admissible.

This facility is for routine care. Emergency attendance rules are different.



REIMBURSEMENT OF MEDICAL CLAIM
 

For OPD treatment, the cost of consultations, radiological, pathological investigations and medicines purchased from the market may be submitted in the form meant for OPD treatment only. The essentiality certificate `A’ as well as cash memos should be necessarily countersigned by the AMA affixing the Hospital stamp.

The claim forms duly completed by the staff members and countersigned by the AMA, with the hospital stamp may be forwarded to Administration Section. For Indoor treatment including the hospital bills, stay charges, Operation & Allied charges, Investigation charges, Cost of Medicines supplied by the Hospital/Purchased from Market etc. may be indicated in the medical claim forms meant for indoor treatment (white form) & essentiality certificate `B’ (green form).

All medical claims should be submitted to Administration within 3 months.Separate claims should be filled in for each patient. Cost of spectacles is not reimbursable. Medicines and other items, which are declared by the Govt. of India as inadmissible, published in the CSMA Rules, will not be reimbursed. Hospital reference of MO/SMO for attending outside Hospitals is a must. Proper break up of charges for various investigations, minor procedures etc. must be provided.
